STATUS FORM FOR THE FALL 2009

This form has to be filled out only if the student is changing hisr meal plan  or changing is on/off campus status and also if they are waiving their health insurance. The deadline for this form to be sent in to the Student Account Office is SEPTEMBER 14, 2009.
