International Student Health Insurance

If you become ill or injured you should seek medical attention from SUNY Maritime College Health Center. The Health Center will refer you to an off-campus provider if necessary. Go directly to the emergency room or call 9-1-1 if you are in a life threatening emergency.

HTH Worldwide is contracted by SUNY to provide health insurance to you while in the United States and abroad.

You are required by SUNY Maritime College to have health insurance for the duration of your stay. Unless you have provided Student Accounts with proof of your own coverage that is comparable to HTH Worldwide, then health costs will automatically be added to your bill each semester.

It is necessary to have health insurance because the cost of health care services here in the United States is very high. If you require medical attention beyond what SUNY Maritime College’s Health Center can provide, it is important you are able to receive the proper attention without accumulating a large amount of out of pocket expenses.

Insurance Plan Highlights

• $200,000 per year for each injury or illness
• $10,000 for accidental death & dismemberment
• $50 deductible (waived if you receive a referral from the Student Health Center)
• Pre-existing conditions are covered
• Dental treatment for accidental injury to sound, natural teeth
• Prescription drug card
• Reasonable expenses for necessary treatment

This plan provides full policy benefits everywhere in the world except in your country of origin. In the participant’s country of origin there is a once in a lifetime emergency only benefit of US$5,000.

Dependents that have enrolled with HTH Worldwide have an identical health insurance plan except for the following:

• $100,000 per year for each injury or illness
• $50 deductible cannot be waived
• Pre-existing conditions are covered only if enrolled within 31 days of eligibility

Health Insurance Card

You must present your insurance card to the off-campus medical provider(s).
Pick your health insurance card up at the Health Center from Ms. Cortney Worrell, Director of Health Services, or print it off of the HTH Worldwide website.
Without your card or insurance information, you will be billed for the services provided.

Referral

If you are referred by the Health Center to an outside provider, it is important that you obtain a claim form that is stamped by the Health Center. This will be your proof of a referral when you visit the doctor. If you have not received the stamped claim form from the college Health Center, you will pay a $50 deductible.

Note: SUNY Maritime College’s Health Center services are unavailable to dependents, and therefore Health Services will not provide dependents with a referral. Your spouse and/or child will be required to pay the $50 deductible per each injury or illness.

Medical Evacuation and Repatriation (MEDEX)

You are required as an international student to have coverage for emergency evacuation and repatriation. The cost is covered within your health insurance premium. MEDEX provides the following benefits:

• Transportation back to country of origin when sick or injured
• The return of deceased member’s remains to country of origin
• Translation services
• Emergency Reunion Benefit
• Political and Security Evacuations

Have your own health insurance?

To use your own health insurance you must determine if it meets SUNY guidelines. You may file a foreign health insurance waiver in the Controller's office (located in the Fort). Attach a copy of your insurance card front and back, along with the effective and termination dates and description of your benefits. You will also need to provide a letter from your insurance company (in English) that certifies you are covered for international travel, in addition to proof of medical evacuation and repatriation. Your documentation will be reviewed, and if it is deemed comparable, you will have effectively waived out of the SUNY International Insurance Program. Be aware of any applicaple deadlines.

Dependents

You should enroll your dependent(s) in health insurance when you have enrolled at SUNY Maritime College or within 31 days of his/her arrival in the United States, date of marriage, or date of child’s birth. To enroll your dependent click here. You must complete the form and send to the address specified, with the documentation and fee(s) that are required.
